Handbook Forms

It is important to read and discuss the student handbook with your child.  After reading and discussing the handbook, please review and sign the forms located in the back of the handbook.  All forms are due on Tuesday, September 8, 2020.  If you are an "at-home learner", bring your forms to the drop-off area from 9:00 a.m. - 11:00 a.m. Forms are on page 56, 60, 61, 62 and on page 64.  Remember the administration of medicine form should be completed by your child's doctor.
